EXPLANATION OF FORMS
CIT Travel: This form needs to be filled out for ALL CITS. This form will let us know your daughter’s travel arrangements to and from camp this summer and if she will need to use camp transportation. Please complete this form as soon as you can. This form can be filled out online or downloaded, printed, filled out and faxed or sent back to camp. It needs to be returned to the camp office by May 1st.

Health Form: This form needs to be completed for ALL CITs and returned to the camp office by May 1st. This form is not complete without the signatures of the CIT’s physician and parent/guardian. As this form requires a signature it is only available in .pdf format for downloading purposes.

CIT Trip Release Form: This form needs to be filled out for ALL CITS. If you would like more information on this trip, please contact Alicia. As this form requires a signature it is only available in .pdf format for downloading purposes. This form needs to be returned to the camp office by May 1st.

Parent Travel: This form needs to be filled out for ALL CAMP FAMILIES, as needed. If you have more than one daughter at camp, you only need to fill it out once.  This form will let us know where you will be this summer and how to contact you. This form can be filled out online or downloaded, printed, filled out and faxed or mailed back to camp. It needs to be filled out and returned to the camp office by May 1st.
